ABSTRACT

Objective: To describe a case of esophageal ulcer associated with the use of alendronate. Case Report: This is the fifth case ever described in the literature according to our bibliographic review. In our patient, the association between the drug and the esophageal lesions was masked by the presence of a hiatal hernia, potentially a cause of the esophageal lesion. The persistence of the lesions despite high doses of anti-reflux therapy called attention to the possibility of the relationship. The esophageal lesion healed soon after suspension of alendronate. Discussion: The authors present a review of the literature and point to the need for diagnostic investigation, to suspend such a drug from patients who experience dyspeptic symptoms while using it.

ABSTRACT

Foram estudados 30 pacientes internados na Unidade de Terapia Intensiva do Hospital Santa Rita de Säo Paulo, com doenças graves. Constituíram-se dois grupos de 15 pacientes cada; um recebeu ranitidina injetável, 50 mg EV de 6 em 6 horas e outro serviu como controle. Foram realizados estudos endoscópicos pré e pós-tratamento, bem como avaliaçäo clínica, laboratorial e eletrocardiográfica. Conclui-se que: 1) É alta a incidência de lesöes agudas da mucosa do trato digestivo alto (LAMTDA) em pacientes gravemente enfermos, internados em CTI (24 dos 30 casos analisados - 80%). 2) A ranitidina, administrada por via endovenosa, mostrou-se altamente eficaz na prevençäo e tratamento das LAMTDA destes pacientes (93% de bons resultados); 3) Näo foram encontrados efeitos colaterais que pudessem ser atribuídos ao uso da ranitidina injetável; 4) Näo se observou interaçäo medicamentosa entre a ranitidina e outras drogas comumente usadas em pacientes graves
